Smartphone app monitoring activity linked to modest short-term reduction in workplace psychological distress
A randomized trial of 793 Japanese workers found that a smartphone app passively tracking physical activity and mental health reduced psychological distress at 3 months, but the effect did not persist at 6 months.
Researchers conducted a 6-month randomized controlled trial in Japan to test whether a stand-alone smartphone app could reduce psychological distress in workers. They enrolled 793 workers aged 18 and older (intervention n=397, control n=396); participants in the intervention group used an app that passively monitored physical activity and mental health for 3 months, while the control group received only a stress-education booklet.
Psychological distress was measured using the 6-item Kessler Psychological Distress Scale at baseline, 3 months, and 6 months. At 3 months, the intervention group showed a statistically significant decrease in psychological distress (mean difference −0.56, Cohen d −0.17, P=.04), representing a small effect size. However, this benefit did not persist at 6 months. Digital activity recordings showed a 7.92-minute increase in the intervention group during the active intervention period, yet self-reported physical activity declined similarly in both groups without significant differences. Subgroup analysis suggested larger effects among participants with minimal baseline distress. The authors concluded that while the app modestly reduced distress in the short term, it remains unclear whether benefits were mediated by increased physical activity, and booster strategies would be needed to sustain effects beyond 3 months.
The study's small effect size (Cohen d −0.17) and lack of sustained benefit at 6 months suggest real but modest impact—roughly equivalent to a 0.56-point reduction on the 6-item distress scale, which has a range of 0–24. Notably, the app increased objectively measured activity by ~8 minutes daily during the active monitoring phase, yet this did not translate into corresponding self-reported activity gains or sustained psychological benefit. The subgroup finding that healthier workers benefited more suggests the app may work best as a prevention tool for low-risk populations rather than as an intervention for those already distressed. The authors note that the mechanism remains opaque: passive monitoring alone may provide mild psychological benefit through increased awareness or behavioral accountability, but whether this works through actual activity increases or other psychosocial pathways is unknown. The lack of a true control app condition (the control group received print materials only) limits causal inference about app-specific effects versus attention or engagement effects. These patterns align with prior work showing that engagement with mHealth tools typically wanes without structured follow-up.
