Moderate physical activity linked to lower insomnia risk, but occupational activity may increase it
A systematic review and meta-analysis of 27 observational studies found that moderate physical activity was associated with a 7% lower risk of insomnia, but high physical activity showed no significant benefit. Notably, occupational physical activity was linked to a 21% higher insomnia risk.
Researchers conducted a systematic review and meta-analysis of observational studies published through August 2024 to clarify real-world associations between different types and intensities of physical activity and insomnia symptoms in the general population. The analysis included 27 risk estimates from studies examining these relationships.
Compared with low physical activity, high physical activity was not significantly associated with insomnia risk (odds ratio 0.96, 95% CI 0.85–1.09), meaning no protective benefit was found at the highest levels. Moderate physical activity, however, showed a modest protective effect, associated with a 7% lower insomnia risk (odds ratio 0.93, 95% CI 0.87–1.00). Subgroup analyses revealed important variation by activity domain: occupational physical activity was linked to a 21% higher insomnia risk (odds ratio 1.21, 95% CI 1.02–1.43), suggesting that work-related activity may strain sleep differently than recreational or leisure-time exercise. The authors found no evidence of publication bias, strengthening confidence in the findings. The results highlight that the relationship between physical activity and sleep is not straightforward—the type and context of activity matter as much as the intensity.
This meta-analysis of observational data reveals a nuanced picture that challenges the 'more exercise is always better for sleep' narrative. The confidence intervals for high PA and occupational PA crossed 1.0 or sat just outside it, meaning these associations are modest in magnitude and not conclusive. Leisure-time and sports activity showed trends toward protection but were not separately quantified in the headline findings.
