Slow yoga breathing and corpse pose reduce anxiety severity and stress biomarkers
A randomized controlled trial of 150 people with anxiety disorders found that eight weeks of slow pranayama (controlled breathing) and savasana (lying meditation pose), added to standard psychiatric care, significantly reduced anxiety symptoms and improved quality of life compared to care alone.
Anxiety disorders involve dysregulated stress responses, inflammation, and metabolic imbalance that erode wellbeing. This 8-week randomized controlled trial assigned 150 people with diagnosed anxiety disorders to either routine psychiatric care alone or routine care plus twice-daily slow pranayama and savasana sessions.
Of 150 enrolled, 112 completed the study. Anxiety severity was measured using the Hamilton Anxiety Rating Scale; quality of life was assessed with the WHO Quality of Life-Brief questionnaire; and researchers tracked serum cortisol, brain-derived neurotrophic factor (BDNF), inflammatory markers, and metabolic parameters. The intervention group showed significant improvements in anxiety scores and across physical, psychological, and environmental quality-of-life domains compared to controls. Serum cortisol (a stress hormone) decreased significantly in the yoga group, and BDNF (a brain growth factor associated with resilience) increased. Inflammatory and metabolic markers trended favorably but did not reach statistical significance after correction for multiple comparisons. One limitation is the modest 75% completion rate and lack of detail on which specific pranayama techniques were taught.
The study's mechanistic findings strengthen the case for yoga as more than placebo: cortisol reduction and BDNF elevation are well-documented pathways linking stress recovery to mood resilience. Notably, inflammatory markers (TNF-α, IL-6, CRP) and metabolic parameters (glucose, lipids) showed favorable directional trends but did not survive multiple-comparison correction—suggesting the effect on systemic inflammation may be modest or require longer observation. Anxiety and quality-of-life improvements correlated significantly with the biomarker changes, indicating the subjective and biological shifts moved together. One gap: the study does not isolate which component (pranayama vs. savasana) drove the benefit, nor does it compare yoga to other active interventions like aerobic exercise or cognitive-behavioral therapy. The sample was 75% compliant, suggesting feasibility in clinical settings but raising questions about whether effects depend on high adherence.
