Online pelvic floor exercises reduce postpartum incontinence but show limited benefit in older women
A meta-analysis of 16 randomized trials found that online pelvic floor muscle training significantly reduced urinary incontinence incidence and symptom severity in postpartum women, but showed minimal effect in elderly patients.
Urinary incontinence is a common problem affecting women's quality of life across different ages. Researchers conducted a systematic review and meta-analysis of 16 randomized controlled trials examining online pelvic floor muscle training (PFMT) in women with urinary incontinence.
In postpartum women, online PFMT reduced incontinence incidence by 44% (relative risk 0.56; 95% CI: 0.38–0.82) and modestly improved symptom severity (standardized mean difference –0.25). However, in elderly patients, symptom severity showed minimal improvement (–0.12, not statistically significant). Exercise compliance improved in both age groups, suggesting that the online format enhanced adherence to training. The analysis found moderate-to-high heterogeneity across studies, indicating variability in study designs and populations. Notably, online PFMT did not produce a measurable difference in quality of life across either group, though compliance gains suggest sustained engagement with the intervention.
The 44% reduction in incontinence incidence in postpartum women is particularly notable because it occurred with an online delivery format—suggesting accessibility barriers were not limiting effectiveness. The compliance improvement in both postpartum and elderly cohorts points to a key mechanism: the convenience of at-home, self-paced exercise removes scheduling friction that plagues in-clinic pelvic floor therapy. Interestingly, the failure to improve quality of life despite symptom reduction hints that other factors (social stigma, incomplete symptom resolution, or measurement timing) may mediate psychological benefit. The limited effect in elderly patients (SMD –0.12) contrasts sharply with postpartum gains and may reflect age-related differences in muscle responsiveness, comorbidities, or hormonal context. The moderate-to-high heterogeneity cautions that individual trials differed substantially in protocol, baseline severity, and follow-up duration—meaning some subgroups may respond better than the pooled estimate suggests.
