However, the protective effect was domain-specific: leisure-time activity (20 studies) showed a 24% risk reduction (RR 0.76), while household activity showed a 15% reduction. In stark contrast, occupational activity (5 studies) was associated with a 20% *increased* risk (RR 1.20, 95% CI 1.01–1.42), and commuting activity (2 studies) showed a 10% increased risk (RR 1.10, 95% CI 1.03–1.18). The certainty of evidence ranged from very low to low across domains, and the high heterogeneity among studies (I² up to 97%) suggests substantial variation in study populations, measurement methods, and follow-up periods. The mechanisms underlying domain-specific differences remain unclear but may involve factors such as stress exposure, occupational hazards, or the volitional versus imposed nature of different activities.
